The purpose of this survey was to collect data on the negative consequences associated with drug and alcohol use. Collected information about: 1) attitudes and beliefs concerning the consequences of drug and alcohol use, 2) individual's experiences and behavior in relation to their own use of alcohol, medications and illegal drugs, 3) other's use of the above substances and the effect on the respondent, and 4) demographic characteristics.
