Cross-sectional postal study among children aged 2-17 years from the 5 Nordic countries; Denmark, Finland, Iceland, Norway and Sweden. A random sample stratified for age and gender was drawn in all the Nordic countries. The total population included 10213 individuals in 1984, 10317 in 1996 and 7715 in 2011.
